What each UTM parameter is for
utm_source names where the visit came from, utm_medium names the kind of channel, and utm_campaign names the campaign the two belong to.
A worked example: a code on a flyer for the spring sale becomes utm_source=flyer, utm_medium=qrcode, utm_campaign=spring-sale. In the Google Analytics acquisition reports that arrives as its own row rather than as anonymous direct traffic.
Two optional ones exist as well — utm_term for paid keywords and utm_content to tell two variants of the same ad apart. Most campaigns never need them, and adding parameters you will not read only makes the URL longer and the QR Code denser.
Naming rules that keep your reports readable
Write every value in lowercase with no accents and use hyphens instead of spaces, because Google Analytics treats "Instagram" and "instagram" as two different sources.
That single inconsistency is the most common way UTM data becomes useless: the same channel splits across three near-identical rows and no total is ever right. Agree the spelling once and reuse it.
Keep utm_medium to a short, fixed vocabulary — qrcode, social, cpc, email, print — so you can group whole classes of traffic later. Save the descriptive detail for utm_campaign, which is where it belongs.
Why tag a QR Code specifically
Without UTM parameters, every scan of a printed code lands in Google Analytics as direct traffic, indistinguishable from someone typing your address by hand.
That is the single reason most businesses cannot say whether print worked. The scan happened, the visit happened, and the report attributes it to nothing in particular.
Tagging with utm_medium=qrcode fixes it in one move, and using a different utm_source per placement takes it further: you find out that the packaging insert outperformed the poster, which is the kind of answer that changes next quarter's budget.

Do UTM parameters change the page that opens?
No. They are read by analytics tools and ignored by the page itself, so a visitor lands on exactly the same content whether the tags are there or not.
Are UTM parameters visible to the visitor?
Yes, they appear in the browser address bar. That is cosmetic rather than a privacy issue, since they describe the campaign and not the person, but it is a reason to keep values tidy and readable.
Do I have to fill in all three fields?
Google Analytics needs utm_source to attribute a visit at all, and utm_medium and utm_campaign make the report useful. In this tool all three are optional — leave them blank and you simply get a plain, untagged QR Code.
